INCOMING RESIDENTS

ONBOARDING INSTRUCTIONS

First and foremost, congratulations! We are so proud and happy to have you join the Brandon EM family!

You will receive a few emails within a week following the official matchday (Friday).

​

Please, be on the lookout for any emails from Medhub!

​

NPI Number (National Provider Identifier Number):

​

This is a required national identification number needed for residency. You DO NOT need to have your diploma to obtain this.

​

Registration:

Step 1: Go to --> https://nppes.cms.hhs.gov/#/

Step 2: Create Account 

Step 3: Login with newly created account

Step 4: Click "Apply for an NPI for myself" under individual provider.

Step 5: Select "No" for sole proprietor question.

Step 6: You may put your domestic billing address and phone as 119 Oakfield Drive, Brandon, FL, 33511 or your home address.

Step 7: Practice Location put as "119 Oakfield Drive, Brandon, FL, 33511" and "813-681-5551"

Step 8: Provider Identifiers - Leave Blank

Step 9: Provider Type Code & Taxonomy - Scroll down to 39 Student in Organized Health Care - Press save

Step 10: Contact Person Name - Enter your information

Step 11: Press submit

Step 12: An email will be sent within 1-5 days (or as little as 10 minutes) with your NPI number.

Step 13: Please keep a copy of this NPI number for your records.
